개요
혼합결합조직병(MCTD, ICD-10: M35.1)은 SLE, 전신경화증, 다발근염의 임상 소견이 중복되면서 고역가 anti-U1 RNP 항체가 양성인 자가면역 질환입니다. Sharp 등(1972)이 처음 기술했으며, 독립 질환 여부에 대한 논쟁이 있으나 특유의 임상 패턴이 인정됩니다.

SLE(나비 발진, 신염), 전신경화증(피부경화), 다발근염(근력 약화) 소견이 시간 경과에 따라 순차적으로 출현할 수 있음.
치료
증상별 맞춤 치료
관절염: NSAIDs, hydroxychloroquine, MTX. 근염: 글루코코르티코이드 ± azathioprine. 레이노: CCB(nifedipine), PDE5i.
폐동맥고혈압(PAH)
조기 발견(매년 심초음파 선별)이 핵심. 확진: 우심도관검사. 치료: ERA(bosentan), PDE5i(sildenafil), prostanoid.
예후
10년 생존율 80-90%. PAH와 간질성 폐질환이 주요 예후 결정인자.
