개요

영상검사

반복 UTI, 남성 UTI, 복잡성 UTI에서 초음파/CT로 구조적 이상 배제.

치료

단순 방광염

1차: nitrofurantoin 5일, TMP-SMX 3일(내성률 확인 후), fosfomycin 단회. 퀴놀론은 단순 방광염에 비권고(내성 보존).

급성 신우신염

경증: 경구 퀴놀론 7일. 중등-중증: 입원 IV 항생제(3세대 세팔로스포린 ± aminoglycoside).

재발 예방

폐경 여성: 질 에스트로겐. 크랜베리 추출물: 일부 근거. 항생제 예방: 연 3회 이상 재발 시 저용량 장기 투여.
