개요

항인지질증후군(APS, ICD-10: D68.61)은 항인지질 항체에 의해 동맥/정맥 혈전증과 반복 임신 합병증이 발생하는 자가면역 혈전성 질환입니다. 전신홍반루푸스(SLE)와 동반이 흔하며, 단독 발생(원발성 APS)도 있습니다.

진단

Sydney 분류기준(2006)

임상(혈전증 또는 임신 합병증) + 검사(aCL/β2GPI IgG 또는 IgM 중등도 이상, 또는 LA 양성, 12주 간격 2회 확인) 동시 충족.

항체 검사

3가지 항체(aCL, β2GPI, LA) 모두 검사. Triple positive(세 가지 모두 양성)이 혈전 위험 가장 높음.

치료

항응고 치료

혈전 APS: 와파린(INR 2.0-3.0) 장기 유지. DOAC는 APS에서 와파린 대비 열등(TRAPS trial) → 비권고. 동맥 혈전: 항혈소판+항응고 병합 고려.

산과 APS

저용량 아스피린 + 예방적 헤파린(LMWH)으로 출산 성공률 70-80%.

CAPS

응급: 항응고+스테로이드+혈장교환+IVIG 병합.
