개요

IgG4 관련 질환(IgG4-RD, ICD-10: D89.89)은 하나 이상의 장기에 IgG4 양성 형질세포가 침윤하고 섬유화가 진행되는 면역매개 질환입니다. 자가면역 췌장염(AIP), 후복막섬유증, 경화성 담관염, 미쿨리치병(타액선/누선 종대)이 대표적입니다.

진단

2019 ACR/EULAR 분류기준

임상(전형적 장기 침범) + 조직(storiform fibrosis + IgG4 형질세포 침윤) + 혈청 IgG4 상승. 조직검사가 gold standard.

혈청 IgG4

>135 mg/dL이면 의심이나 특이도 낮음(60-70%). 정상이어도 IgG4-RD 배제 불가.

치료

글루코코르티코이드(1차)

Prednisolone 0.6mg/kg/일 × 2-4주 → 감량. 대부분 극적 반응(97%). 재발률 30-50%.

스테로이드 절약제

재발/스테로이드 의존: azathioprine, mycophenolate, rituximab.

Rituximab

난치성/재발성 IgG4-RD에서 우수한 효과. B세포 고갈로 IgG4 생성 감소. 관해 유지에도 활용.
